Travel Vaccinationsglobal air travel

If you require vaccinations relating to foreign travel, please complete the Travel Questionnaire form below and submit to the surgery with payment for your initial assessment/consultation.  Please contact the surgery on 01386 793007 for details.

Upon receipt of your travel questionnaire and payment, the practice nurse will assess your vaccination requirements by reviewing your electronic and paper notes where necessary.  She may contact you if any further information or a face-to-face consultation is required.

Following the initial assessment/consultation you will be contacted by a member of the admin team to book your appointment to receive the vaccinations.  Payment for certain vaccines will be requested at this point.  Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Please be aware that you may require a course of up to 3 doses of certain vaccines.  The last vaccination you receive should be at least 2 weeks before you travel in order for the vaccine to work.  Therefore please allow a period of at least 8 weeks before you travel for your course of vaccinations.
